The Game Gear Puyo Puyo was, like its Mega Drive counterpart, released outside Japan in modified form as Dr. Robotnik's Mean Bean Machine. This poor series just couldn't catch a break.

English Mode
The game loads an English mode when played on non-Japanese systems, suggesting that a direct English-language release had originally been planned.

- The name is changed to "Puzlow Kids", a title localization unique to the Game Gear version.
- Puyos are called "P-Kids", as the title suggests.
- The English script is complete and nearly identical to the English arcade version. This includes the two endings (which also includes a mention of "Dark Prince", an alternate name for Satan that is used in most English releases), the difficulty select menu, and the "All Right!" and "Oh no..." graphics for Vs. Mode.
Error Screen
An error screen prompting the user to "check cartridge", which is used when the link cable is tampered with during a 2-Player game. It can also be seen by editing address D401 to any value higher than 03 in a memory editor when on the main menu, which will cause the screen to appear when pressing Start. After a few seconds of displaying this screen, the game will forcibly reset itself to the Sega logo.
Notably, this screen uses graphics seen nowhere else in the game. The artwork is taken from the title screen of the Genesis version.
A similar screen is present in Dr. Robotnik's Mean Bean Machine.
