About the Unreachable Medipack's existence

Showcase a video, where the "Unreachable Medipack" can't be reached due to the incorrect pillar collisions, and what exact game version (platform and revision) has both the medipack existing alongside the incorrect collisions (where both are true). "Second PS1 version" is ambiguous and there are no linked materials presenting the occurrence on PS1 (I've never seen this happening despite playing multiple PS1 revisions).
