Gameplay Differences
- The game now runs in widescreen. As a result, some of the boss battles and arenas are altered, such as in Aquatic Ruin Zone and Death Egg Zone (Mecha Sonic only).
Graphical Differences
- Smooth sprite rotation has been implemented for certain sprites that rotated in the original version. (e.g. the drills of Mystic Cave Zone's boss and the cogs in Metropolis Zone).
- Many other objects have also had their animations smoothed, such as the Rexon badnik.
Sega Screen
In the original Genesis version, Sonic's in-game sprite is used and enlarged accordingly. The 2013 version uses new upscaled Sonic sprites for the Sega screen.

Audio Differences
- All of the music uses cleaner instrument samples than the original version.
- The only non-level music to have a sped-up version is the invincibility theme. One effect of this is that encountering a boss with Speed Shoes will only play the normal boss theme, not the sped-up version.
- Hidden Palace Zone uses the 2-player Mystic Cave music assigned to it in various prototypes of the original game, with Song #10 (the music assigned to Hidden Palace in the original game's final release) instead used for Proto Palace Zone.
- The Wing Fortress Zone boss now has sounds for charging up and firing its laser, taken straight from Sonic 3 & Knuckles.
- Death Egg Zone also has new sounds for closing the Death Egg Robot's head and explosion sounds when escaping, also taken from Sonic 3 & Knuckles.

Chemical Plant Zone
Act 1
- There are two new secret monitors near the start of Act 1:
- The two monitors that were introduced in Knuckles in Sonic 2 are present:
- There's a whole new path exclusive to Knuckles, as well as a Knuckles-exclusive checkpoint area:
Act 2
- The entire second half is filled with new Knuckles-exclusive paths.
Hill Top Zone
Act 1
- There is now a brand-new Knuckles-only section near the beginning:
- There is also a new Knuckles-only section near the entrance to the first underground section:
Mystic Cave Zone
Act 2
- The infamous spike pit has all of its spikes removed, though jumping down there isn't treated like a bottomless pit - it instead sends you to Hidden Palace Zone!
Wing Fortress Zone
- The lone diagonal conveyor belt now actually functions as a conveyor belt, unlike the original where it served no actual purpose.
- Speed boosters were tweaked and are able to launch the player across the large gap far in the level.
