This near final demo of MediEvil, dated September 16, 1998, was distributed to gamers in the mail through the PlayStation Underground and was also on the 1998 Pizza Hut demo disc.
It is closer to the final version than the previous demo, though there are some audio differences and certain minor visual tweaks. The following levels are playable: Dan's Crypt, The Graveyard, Cemetery Hill, The Hilltop Mausoleum and The Hall of Heroes. The demo ends right before the boss fight with the Stained Glass demon, though there is a time limit before the demo resets/ends.
Audio Differences
- The title sequence, level map screen and the Hall of Heroes level all use an excerpt from the music for the Crystal Caves level, though it is dialed back slightly compared to the final.
- The intro music in Dan's Crypt, leading into the gargoyle conversations is different, using an brief musical excerpt from the Return To the Graveyard level.
- Dan's Crypt and The Graveyard play an excerpt from the correct music from the final version. The same excerpt is used for The Hilltop Mausoleum level as well.
- The music in Cemetery Hill plays an excerpt from the Scarecrow Fields level.
- The musical flourishes that open the levels are missing.
- Full voice overs have been implemented.
- During the battle flashback in the intro lead-in to Dan's Crypt, Zarok is missing his line where he says, "Fire!"
Miscellaneous Differences
- The vibration function and L3 walk/run toggle are fully implemented.
- The pointer arrows on the map screen that surround Dan's head are white instead of the final game's green. The restricted red arrows are missing as well.

- The images from the previous demo that explain the game's features now have a fade in/out effect.
- Since there is no opening movie or opening text crawl in the demo, the game replays the intro lead-in to Dan's Crypt when left on the title screen.
